Tibia in 3D - How would you design it?

Lets say that you've been hired by CipSoft to design a "new" Tibia in 3D.
Same game (plus/minus a few old and new things), but in 3D so that they can stop having to pay so much money for professional sprite artists for their weird perspective and can just hire regular 3D artists and throw Blender at them.

What do you think is important to keep gameplay-wise? And what could be thrown out?
Would you use First-Person perspective like Minecraft, or go with a more 3rd person camera WoW approach?
Would the graphics be fancy, or would it look like Runescape? Or maybe even blocky like Minecraft?
Would you revamp the spell system, or go back to the old spoken spells with tile AOE's and targeting by name?

How would you do it? What would be your vision?
 
Tibia 3D para mim seria no mesmo estilo de world of war craft, porém, com a história e missões do tibia.

If it's not some sort of isometric view with a grid for movement it would be a too different game to be called Tibia.
