AAC Bans problems

so basically when i try to ban somebody on my srever it says Account is already banned and there becomes a error in my forgottenserver saying like mysql_real_query INSERT INTO 'bans' ('id', 'type', 'value', 'password')

and a bit of other stuff anyone can help?

If you could print the error it would be better.
