What program do i use to create items and weapons from scratch?

Do you mean paint them? If so, you will need something like sprite editor. However, you will have to use custom client if you want custom sprites in your ot.

For new sprites (graphic part)
SPR Editor

And for the items.otb
OTItemEditor (for 8.5)
