error when creating an account from the account manager

hello otland people I have a small problem with the account manager, they cannot ask me for permission to create it, why?

i am using otx 2.15

Type: voidQuery (voidQuery is used for update, insert or delete from database) Field 'comment' doesn't have a default value When i try to create new character.
