How to do script with lever and showing tp

Yo,
I want to do quest. First someone have to use lever on location "X" and then he can go to location "Y" when TP will be Showed. If someone will not use lever, TP will be hide. How to do this script??


btw, would be great like: someone go in tp and then this teleport will hide again! :)
