Yes its possible, search for things like otitemeditor or spriting etc.

If you want to use custom item sprites, your players will have to download custom client

Items is not hard to make, but if you want new sprites, you have to make (edit) a client (witch isnt to hard). And I prefer just edit the scripts, instead of using a itemeditor. Pretty sure thats easier to stuff like this
