Server crash - ban table error?

Hello! My server is crashing a lot, and today i saw this error in the console:

I'm using TFS 0.3.5 (crying damson).
What this could be? I will be thankful for any help!

Yep here is problem.
You should delete it and change this restarter for another one better.
