how does it works with outfits ?

You need to make different outfit like: he looks to south and right foot is in front and left foot is behind, he looks to south and right foot is behind and left foot is in front, etc. i think.

Get a spr and dat editor for your version and replace existing sprites with that as ur outfit then just edit them in the dat.
