Died by FIRE,ICE?

I've got something wrong with my server and i don't know the problem until now :S, when someone die by monster or player i found this

You see a dead human (Vol:10).
You recognize Utori. He was killed by ice.

You see a dead human (Vol:10).
You recognize Utori. He was killed by physical.

but i already died by a monster.

Does monster use ice? If monster hit with mele then physical is correct.

It isn't normal lol, it must be like that NAME killed by MONSTER NAME or PLAYER NAME, so there is something wrong and i don't know what is it.
