Error In create Character with MyAcc

Hello, I have an error when I want to create a character with the myacc site, it is the first time I implement it and I don't understand it very much... I always use gesioracc but the problem is the following, it seems I have everything configured but I can't Create the character, I don't know where the error could be. can someone help me?
I have the samples configured correctly in the config and in the database.

It is not a problem with the capitalization, in the image it is written wrong because I was trying everything
