"Full" security but hack ? help!

Hello guys!

I've got this security:

-repair ots_mysql pot
-phpmyadmin ONLY from localhost

And i've been hacked. I think it was phpmyadmin hacked.
Why? I haven't got keylogger or whatever.

Is more security?
