You can use Noxitu's sprite editor to add new sprites/items into Tibia.spr/Tibia.dat for client, and OTItemEditor to add them into the server's items.otb.

do i need to upload the tibia.spr and tibia.dat to make other ppl see the items?

It depends, if you alter the IMAGE of the item, yes, they do, but if you alter attack, name and stuff that don't mess with how the items looks like in-game, then no, they won't have to download your tibia.spr or .dat
