Windows I can't Registre more 2 account

Hi, sorry my english... i install de Web Serv all This OK!.

when a user enters the page and then try to create an account, enter the data without problems, but nothing appears the database and the user is logged in my database appear only 2 registered users (GM, and another account ) if I delete the second account, I can create, but after 3 accounts I can not create more accounts web page...

On page gives me no errors, and the database only allows me 2 records of accounts.
