Do you got SQLITE or Mysql?

Open your SQLITE/MySQL and go to Players/Account and look at the right side you will see.

It saves in database: db_server_name > bans > (find the player/account you bannished him/her ip; also, the type can show you which type of bann did you use).
I don't remember which type is what...

If you want to unban you can just do it by a command ingame
!unban >.<

It's not !unban, it's:

/unban playername
