Item bug TFS 1.3

Hi. I have bug with some items when i wear it and relog this happens.


Does anybody know how to solve it? Because I cant put out this cyclop on my head so always I must delete char and make new one.
This happen for example with item Galea Mortis. I have item in my client, items.otb, items.xml and in movements.xml also.
P.S: Where Can i Delete in database player Equipment?

@Evil Puncker ye but this is only page I dont know edit because there is no option to delete. :/

DELETE FROM `player_items` WHERE `itemtype` = 0000

change 0000 to the item id and execute the query, it should work
