If you try to open a custom items.otb file with a normal client which doesn't have custom sprites implemented.

nop, i tryed to open custom items.otb, with custom client too

well, now i have the imagens... but no descriptions

One thing you could do is open your custom client with Object Builder and check if someone have defined properties for those items. For example if the item is movable, pickuple, container etc.
If you are looking for item descriptions which are like "This item gives you +3 magic level" . Then you should take look at the items.xml file where the item armor, attack, skill bonuses and element resists are written down.
