You have dublicate UID's and missing 2 lua files in talkactions.
It's not a serious problem. Your server will run fine without editing anything.

Anyhow;
Go to /data/talkactions/ and open talkactions.xml
Remove:

<talkaction log="yes" words="/squelch" access="3" function="squelch"/>
<talkaction log="yes" words="/cliport" access="3" function="clickTeleport"/>

Are you sure you are visible? Remember that you can see yourself, but other players can't see you.
To give banishment or notations, right click the player and choose Rule Violation. Then choose desired action against player.

Ban length and notation length can be configured in config.lua

There is no ghost.lua, it is a function. So there is something wrong with your distro then.
