Well I would go for costum nowadays. I currently got freaking bored, playing cipsoft maps, but be sure you are a good mapper and that you do not map tiny huntpieces.

I think a lot of people say custom, but they don't play custom.
A real map will definitely bring players, almost all the time if you know what you are doing.

I want to say real map only because I play Tibia and have for a long time. If there was a nice custom, I would play it as well.
