Request subject: Lever script

Good afternoon,

I've been searching for sort of a transforming script, have been searching for that script for quite some time and the ones that I found din't really fit to what I want.

So whats the purpose of the lever script?
Well I can't really explain this but what I need is a script where I need certain items to be placed on certain positions (on a table in front of me or smth like that) and when I pull the lever it transforms in another item.
Also where I can set up multiple items so lets say I want to create a MPA, I throw 5 blood herbs and a blue gem on a certain destination and when I pull the lever is MPA, and when I want to create coconut shoes I need 50 coconuts and a blessed wooden stake or w/e.

I hope you guys can help me out with something like this,
