reset to level 1 bug

hello i have a ot with some players

the problem is that randomly some player if he dies get reseted to level 1

like some1 that was level 187 , died in a qeust and reseted to level 1

and another guy last night he was level 275 and now rested to level 1

Please can some1 help me withs thiss bug !

We need more information, like the server version aka distro, when this 1st started, has it always existed etc etc
