Instanced dungeons are somewhere in otland, you just have to dig deep enought.
Cinematics can be easily done in otclient.
And to create game, you can download any free graphical environment such as XNA and learn c++ or c# a bit and then it's downhill.

About cinematics and my own game, I've been trying to learn more c++, not so easy with limited time. I need some more time to learn more about OTClient =) and I've tried a lil bit of BYOND to make a game hehe.
