I like to start making towns from where the player starts and imagine where they would go from there.
Seems to help me.

First, I always pick a style - snowy, desert, a capital city, mountainside city, mountaintop city, go on..
After that I make the walls. Thick and higher walls for bigger cities, smaller walls for villages etc. Don't forget to make a badass looking entrance/gate.
After that just place houses and stores, churches/temples, stables, farms (I place them outside the gates), depot, maybe a market or park.
