It happened to me too once. You used mana shield at the same time you got hit or the server lagged.

it's none of the reasons you said. It's a big in the new server engine and it will be fixed in the next version. It happed many times in my server, at first I didn't either believe it, but I tried it myself and I think its when you type utamo vita and ur using "mana shield" it gets messed up.
