There are few tutorials how to do this, but mine is best explain (i guess so) and with pictures.
Credits to: Noxitu(i'm not sure who's that, but .spr and .dat editor is called noxitu.)

Ok, follow my steps, and you will successful add your items!
1. Step: Download .spr and .dat editor
Well i found this on google, i can't open virusscan.jotti.org or virustotal, so if someone can scan this 2 .exe files. My antivirus don't show anything, but i never scan it before on online virus scaners.
1. Download Noxitu Sprite Editor, Noxitu Sprite Editor 1.3.0 Download
2. Download Noxitu Items Editor, Noxitu Items Editor 0.9.6 Beta Download
3. http://otland.net/f19/otitemeditor-8-54-a-72114/
2. Step: Run sprite editor, and add new sprite!
Ok, first run sprite editor,
Then press load items
Make sure you choice pink color
Click on blank field and press Add Sprite
Your new item have to be 32x32 size, and background need to be pink!
After you successfull add new item, press save
Remember number of your new file (3258x..)
Ok, you are now done with .spr file! Now we have to fix .dat
3. Step Run and add item into to .dat file!
Load Tibia.dat file
Now put there number of your spr file!
Remember item number, you can see there
Save it, and you successfull imported new item!
Now you have to import it into item.otb file!
